Gold Plan
$500 per person ($650 value)
- One Oral Cancer Screening

- One Periodic Exam
- Two Adult Cleanings per year
- Two Fluoride Applications per year
- 4 Bitewing X-rays (once a year)
- One Panoramic X-ray or FMX (once in 3 years)
- One Oral B Toothbrush (at initial plan purchase)
- Discount of 20% on all services for one year
Platinum Plan
$1,000 per person ($1,875 value)
- One Oral Cancer screening
- One Complete Exam
- One Periodic Exam
- Four Adult Cleanings per year
- Two Fluoride Applications per year
- Four Quadrants of Periodontal Scaling
- 4 Bitewing X-rays (once a year)
- One panoramic X-ray or FMX (once in 3 years)
- One Electric Oral B Toothbrush
- Discount of 20% on all services for one year
